Please fill out and fax the following forms: Parent/Student Insurance Information Form, Parental Permission Letter, and College Claim Form. Fax to (417) 268-6694 to the attention of Mark D. Hedger.

Baptist Bible College athletes must have the aforementioned forms signed by you and your son or daughter for the 2010-11 school term before being allowed to participate in any sport.  This includes being allowed to participate in any tryout or practices. Please read and sign the applicable wavier and insurance forms, then mail or fax it back to the Athletic Department. Thanks for your cooperation in this matter and we look forward to a great season.
